I have spoke to Muppet about this and we are in agreement.

Formation 442

defence war/bbk

left midfied offensive (neo)
right midfield offensive (fuse)

Midfield wingers need to be behind strikers, ideally to get a pass back from strikers so they can slot the ball back through, tapping Y.
we need to stop our players running into each other or tackling each other in the box, it happens far to much.

Centre Right Defensive (steve)
Centre Left defensive (Danny)

Through ball maestros (recieve ball from defence and either goto wingers or hold Y for long through ball or LB+Y for lobbed through ball. (its important you guys dont get caught in possesion) go to midfield wingers if through ball isnt working out.

2nd game midfielders swap roles- hopefully this will remove the urge to want to get forward all the time midfield.

I'm in, and the formation is pretty much what i would have expected (ie: good ideas), except maybe think about having 1 offensive and 1 defensive central midfielder if things are not working out, or we need to push to an attacking formation.

Muppet, not sure what youre going through mate, but i'm just up the road so please let me know if i can help at all.
